The most successful self-employed sports therapists and musculoskeletal professionals don’t build complicated systems... they keep it streamlined and master one.


If your private clinic feels overwhelming, inconsistent, or stuck, the problem probably isn’t that you’re doing too little or your system isn't complex enough.


It’s more likely that you’re doing too much and it's too complicated.


Decision fatigue is one of the commonly missed barriers to clinic growth...


Every morning you wake up and face a mountain of choices: which social media platform to focus on, which type of patient to attract, how to follow up with enquiries, which software to use. Over time, the weight of those unresolved decisions drains your energy, stalls your progress, and keeps you spinning your wheels instead of seeing patients and growing revenue.


The solution is deceptively simple: choose ONE.

This is the principle that underpins how the most confident, fully booked sports therapists and musculoskeletal practitioners operate. Not ten strategies running simultaneously to achieve the goal. One clear, consistent, deliberate choice in each area of their business, executed exceptionally well.


Why is decision fatigue killing your growth?


The more decisions we have to make, the lower the quality of those decisions becomes. For self-employed MSK professionals this is a significant problem.


You’re not a marketing agency. You’re not a tech company. You’re a skilled practitioner who should be spending the majority of your mental energy on your patients and your craft.


When you try to maintain a presence on Instagram, TikTok, Facebook, LinkedIn, and YouTube simultaneously, you don’t do any of them well.


When you offer physiotherapy, sports massage, rehabilitation programmes, online consultations, and group workshops all at once without a clear lead offer, patients don’t know what to book.


When you use three different booking systems and two note-taking apps because you haven’t fully committed to one, you create friction for yourself and for your patients.


Simplicity isn’t laziness. It’s strategy.



Using the 'Choose-One' framework can transform your business. Let's have a look at it in action...


1. ONE Ideal client avatar (ICA)


Often sports therapists and musculoskeletal professionals try to appeal to everyone.


Your ideal client avatar is the specific person your clinic is built for. Not “anyone with pain.” Not “athletes and office workers and elderly patients and weekend runners.” One person. One set of problems. One set of goals.


When you know exactly who you’re speaking to, such as, a 35–50 year old recreational runner dealing with persistent lower limb injuries who wants to get back to marathon training, your entire business becomes sharper.


Your content speaks directly to them.


Your offers are built around their specific needs.


Your messaging cuts through the noise because it sounds like you’re reading their mind.


Defining your ONE ideal client avatar is the foundation of everything else. Without it, your marketing will always feel scattered.


Action step: Write a one-paragraph description of your single ideal patient. Name them if it helps. Describe their injury, their goal, their frustration, and what they’ve already tried.


2. ONE social media channel


You do not need to be on every platform. You need to be excellent on one.


For most sports therapists and MSK professionals, I believe that Instagram remains the highest-leverage platform. It supports video, written content, direct messaging, and referral-style sharing, all in one place.


But the platform matters less than the principle: pick one, go deep, and build an audience there before expanding.


Spreading yourself across multiple social channels is the fastest way to burn out and produce mediocre content everywhere. One platform, posted to consistently with genuine value, will always outperform five platforms managed half-heartedly.


Action step: Audit your current platforms. Which one drives the most engagement or enquiries? Commit to that one for the next 90 days and pause the rest.


3. ONE lead magnet


A lead magnet is a free, high-value resource you offer in exchange for a potential patient’s email address or attention. It might be a guide, a checklist, a workshop, or a self-assessment tool.


Most clinic owners either have no lead magnet at all, or they have five partially finished ones sitting in a Google Drive folder somewhere.


You need one. One lead magnet that speaks directly to your ideal client’s most pressing problem, delivers genuine value, and positions you as the expert they want to work with. (Plus at the end - let's them know the service you offer and how to get it)


For an MSK professional targeting runners, this might be a “5-step return to running assessment” guide. For a sports therapist working with gym athletes, it might be a “Shoulder pain self-screening checklist.” It doesn’t need to be long or complex. It needs to be specific and useful.


One lead magnet, promoted consistently, will build your email list, warm up cold audiences, and generate inbound enquiries on autopilot.


Action step: Identify the single most common question your ideal client has before they book with you. Create a one-page PDF that answers it.


4. ONE direct message script


When someone enquires about your services via direct message, what do you say?


If your answer is “it depends” or “I just work it out as I go”, you’re leaving bookings on the table and you can't test things out.


A single, refined DM (direct message) script is a consistent framework for how you respond to enquiries. It's great because it removes guesswork, speeds up your response time, significantly improves your conversion rate, and reduces decision making (like we spoke about at the start). You’re not reading from a script robotically; you’re following a structure that has been proven to work.


A good DM script for a sports therapist or MSK practitioner will: acknowledge the enquiry, ask qualifying questions, briefly explain how you can help, and invite the person to take a clear next step (e.g. book a call, book a session, visit your booking link).


One script. Refined over time. Used consistently.


Action step: Write out your ideal response to a new DM enquiry. Test it with the next five enquiries you receive. Refine from there. If you want a ready-made script, you can access one with my premium coaching membership.


5. ONE core offer or package


This is where many talented MSK professionals lose potential patients entirely.


When your website lists ten different treatments at varying price points with no clear recommendation, patients don’t know where to start. Confusion kills conversion.


People default to inaction when they’re overwhelmed with options.

You need one front-door offer, your signature package or programme that you lead with consistently. This is the primary way new patients work with you. Everything else (individual sessions, specialist treatments, group programmes) sits behind that front door.


For a sports therapist, this might be a six-week rehabilitation and performance package. For a musculoskeletal practitioner, it might be a structured assessment-to-discharge programme. Whatever it is, it should be clearly defined, clearly priced (or clearly explained in a consultation), and the natural answer to “how do I work with you?”


One core offer removes complexity for your patients and makes your marketing infinitely easier.


Action step: If someone asked “how do I work with you?”, what’s your single best answer? If you can’t answer in one sentence, your offer needs simplifying.


6. ONE booking method


How do patients book with you?


If the answer involves multiple phone numbers, a DM, a third-party booking site, a direct email, and a WhatsApp message then you probably have a problem (especially if you are a sole practitioner). Multiple booking routes create confusion, missed enquiries, a perception of disorganisation that undermines your professional credibility, and a heavy burden on yourself to keep up.


Choose one primary booking method and direct all traffic there. This might be an online booking system, a single booking link, or a consultation call... but it should be one clear, consistent pathway.


One booking method means no dropped balls, a smoother patient experience, and a business that feels effortless to interact with.


(For coaching, mine is to direct people to book a free connection call with me. For clinic it is to book through a link)


Action step: Audit your booking touchpoints. Pick one as your primary. Update your social media bio, website, and email signature to reflect it.


7. ONE booking and note-taking system


Clinic management software is a highly personal choice, but the principle is the same: pick one and use it properly.


Whether you use Cliniko, Jane App, WriteUpp, RehabGuru, or another platform built for MSK and sports therapy professionals, the goal is integration. One system that handles appointment booking, patient records, clinical notes, and ideally invoicing.


Not a patchwork of different tools that don’t communicate with each other.


Using one integrated system reduces admin time, reduces the risk of clinical errors, and creates a professional patient journey from first booking to discharge.


The software itself is less important than the commitment to using it fully. Most practitioners use 20% of their system’s features and then add other tools to compensate for the gaps they haven’t explored.


Action step: List every tool you currently use for bookings and notes. Identify whether one of them could do the job of all of them. Commit to learning it properly.


8. ONE coaching platform


If you’re investing in business coaching or professional development, and if you’re serious about building a fully booked clinic, you should be... work with one programme or one mentor at a time. (I don't mind if it is me or someone else, but please just choose one).


Buying multiple courses, jumping between group programmes, and consuming content from ten different coaches simultaneously is one of the most expensive forms of procrastination available to self-employed practitioners. You gather information without applying it. You start frameworks without finishing them. You stay busy without moving forward.


One coaching relationship. One methodology. One set of frameworks, applied consistently over time.


This is how the practitioners who go from unpredictable income to fully booked clinics actually do it. Not by consuming more, but by committing to implementing one system properly.


Action step: If you’re currently working with multiple programmes or coaches, identify the one that is most aligned with where you want to go. Commit to that one for a defined period before considering anything else.


Successful practitioners aren't doing more than you. They’re doing less, but doing it better. Every decision reinforces the same direction. Every piece of content speaks to the same person. Every enquiry goes through the same process.


The result? A clinic that feels consistent, professional, and easy to run (and patients who feel confident they’re in the right hands).


Common objections to the choose-ONE framework...


Won’t I miss out by only being on one platform?


You’re already missing out by being very average on five. One excellent platform will always outperform five average ones.


What if my ideal client changes?


It will evolve. That’s fine. But you need to start somewhere. Vague marketing to everyone converts no one.


I have too many different types of patients to have just one offer.


Your front-door offer doesn’t have to be your only offer. It’s just the primary entry point. Complexity behind the scenes is fine. Complexity in your marketing is not.


I’m not ready to commit to one system yet.


This is the exact thinking that keeps practitioners in permanent indecision. Commitment creates clarity. Clarity creates progress.


The bottom line...


Building a successful private clinic doesn’t require more strategies, more platforms, or more tools.


It requires commitment to fewer, better choices executed consistently over time.
